Pathfinders is a unique kind of organization. We’re a “village,” a community of providers who work together to address the needs of the whole patient.
How do we do this? We pick up where the medical system leaves off. We understand that even in the best of situations, your medical team is limited in terms of the support they can provide. Much of the work involved once someone is diagnosed happens at home: carrying out a treatment plan, finding solutions to new challenges, trying to live the best possible life. Anyone who has dealt with a difficult diagnosis knows just how challenging it can be to cope on a daily basis.
How are we different? While some organizations offer patient advocacy services or patient care management, we cover a much wider spectrum of services not typically found under one roof. Our goal is to ensure continuity of care leading to the best medical outcomes possible, whether it’s based on a biomedical or complementary medicine approach.
